Pantoprazole is a proton pump inhibitor that reduces the amount of acid the stomach makes. The 40 mg tablet is the standard adult strength: once a day for erosive esophagitis, twice a day in hypersecretory states. The 20 mg tablet covers milder reflux, including short-term heartburn and the step-down dose chosen once symptoms are controlled. Both are swallowed whole before a meal.
How Pantoprazole Works
Pantoprazole is a proton pump inhibitor. It becomes active only in the acidic space of the stomach's parietal cells, where it blocks the proton pump that produces acid. Because it switches off the final step of acid production, it suppresses acid more completely than an H2 blocker. Acid control is only one part of digestive care: nausea may be treated with Domperidone, a shortage of pancreatic enzymes is corrected with Pancreatin, and some liver and bile conditions call for Ursodeoxycholic Acid.
Dosage & Administration
Adults usually take one 40 mg tablet once a day, about an hour before a meal, for up to 8 weeks while erosive esophagitis heals, then 40 mg once a day to keep it healed. For ordinary heartburn and acid reflux the usual dose is 20 mg once a day. Swallow the tablet whole with water. If you miss a once-daily dose and the next is less than 12 hours away, skip it; never take two doses at once. If symptoms do not improve within 2 weeks, see a doctor.

Pantoprazole lowers the amount of acid the stomach makes. It is used to treat heartburn and acid reflux, to treat and prevent gastro-oesophageal reflux disease (GORD), and to heal and prevent stomach ulcers. It is also given with antibiotics as part of ulcer treatment.
Most people have no side effects. Uncommon effects, in less than 1 in 100 people, include headache, diarrhoea, nausea, constipation, a dry mouth and tiredness. Rarely it can cause liver, kidney or bowel problems, or a sun-sensitive rash with joint pain. Used long term it can lower blood magnesium and may raise the risk of fractures and vitamin B12 deficiency.
For heartburn and acid reflux the usual dose is 20 mg once a day, and for GORD or a stomach ulcer it is 40 mg once a day. Erosive esophagitis is treated with 40 mg daily for up to 8 weeks, then 40 mg daily to keep it healed. Take it about an hour before a meal.
Yes. Pantoprazole can affect, or be affected by, antifungal medicines such as itraconazole, methotrexate, HIV medicines, rifampicin and blood thinners such as warfarin. It can also reduce the absorption of some drugs because it lowers stomach acid. Do not take St John's wort with it.
There is no specific interaction between pantoprazole and alcohol, and the label does not advise avoiding it. Alcohol can still irritate the stomach lining and worsen reflux, heartburn and indigestion, which works against the reason you are taking the medicine.
If you bought pantoprazole without a prescription, you can usually take it for up to 2 weeks; if symptoms have improved you may continue for another 2 weeks, but do not go beyond 4 weeks without seeing a doctor. On prescription the course may last weeks or months.
