How Dexamethasone Works
Dexamethasone is a synthetic copy of cortisol, the hormone your adrenal glands produce. It binds to glucocorticoid receptors inside cells and switches down the genes that drive inflammation, so swelling, heat and pain ease and immune activity is suppressed. In an asthma attack an inhaled reliever such as salbutamol opens the airways within minutes, while a systemic steroid is kept for flare-ups that need more than an inhaler. Because it damps immune defences, it can also hide the signs of an infection. Herbal supplements such as Septilin are not tested with dexamethasone, so they cannot be called safe alongside it.
Dosage & Administration
Adults usually take 0.5 mg to 10 mg a day, and up to 16 mg a day for some conditions. Take the tablets in the morning with or straight after breakfast, never on an empty stomach; if the plan is two or three doses a day, take the last one before 6pm. If you miss a dose, take it when you remember unless the next one is nearly due, and never take two doses at once. Do not change the dose or stop early on your own: longer courses are always reduced gradually under medical supervision.
Dexamethasone is a prescription medicine in most countries. Do not start, change or stop it without medical advice, and never stop it suddenly: withdrawal can cause severe tiredness, weakness, body aches and joint pain. Daily doses of 6 mg or more, and courses longer than a few weeks, need regular monitoring by a doctor. This information is not medical advice.

FAQ
Before you ask
Questions, answered
Dexamethasone treats problems driven by inflammation or an overactive immune system: severe allergic reactions such as anaphylaxis, severe skin conditions, inflammatory and autoimmune diseases, asthma flare-ups and croup in children. It is also used for certain blood, eye, thyroid and intestinal disorders, for sickness and vomiting, and to ease some side effects of cancer treatment. It is not a treatment for ordinary colds.
More than 1 in 100 people get weight gain, indigestion, trouble sleeping or mild mood changes. Serious effects are more likely at 6 mg a day or more, or beyond a month of treatment: infections, adrenal problems, high blood sugar, low potassium, a puffy rounded face with upper-body weight gain, eye problems and thinning bones. Get urgent help for breathlessness, swelling, black poo or vomiting blood.
Adults usually take 0.5 mg to 10 mg a day, and up to 16 mg a day for some conditions. It is normally taken once daily in the morning with or straight after breakfast, not on an empty stomach, so it is less likely to keep you awake. Follow your prescription exactly.
No. Stopping abruptly can cause loss of appetite, stomach upset, vomiting, drowsiness, confusion, headache, fever, joint and muscle pain and weight loss, and after long treatment it can leave you severely tired and weak. After more than a few weeks your doctor will taper the dose over several weeks so the adrenal glands can recover.
Yes, alcohol is allowed with dexamethasone and it does not cancel the medicine's effect. Even so, drinking can add to stomach irritation and sleep problems, which are already common side effects. If you have a history of stomach ulcers, or take high-dose aspirin or anti-inflammatory painkillers, guidance is to limit alcohol, because the combination raises ulcer risk.
Yes. Rifampicin, rifabutin, carbamazepine, phenytoin and phenobarbitone can weaken its effect, while antifungal medicines, macrolide antibiotics and HIV medicines can raise its levels and side effects. It can strengthen warfarin, add to potassium loss with diuretics, and increase ulcer risk with anti-inflammatory painkillers. Live vaccines must be avoided.
